Registration & Notarization Requirements

Opticianry is a regulated health profession. As such, every practicing optician in Ontario (including intern and student opticians) must be registered with the College of Opticians of Ontario.

Applying for a Certificate of Registration: Notarization Requirements

When applying for your Certificate of Registration, some of your supporting documents must be notarized.

  1. Verification of Your Legal Name: To verify your legal name, you must submit a notarized photocopy (certified true copy) of your current passport or driver’s license. Alternatively, you may submit a notarized photocopy of your birth certificate accompanied by a second piece of identification bearing your legal name, such as your health card.
  2. Verification of Canadian Citizenship or Immigration status: if you are a Canadian citizen, you must submit a notarized photocopy (certified true copy) of your current Canadian passport. If you are not a Canadian citizen, you must provide evidence of your permanent residency or employment authorization under the Immigration and Refugee Protection Act, such as a notarized photocopy of your current work permit or permanent resident (PR) card.
  3. Statutory DeclarationIf you are unable to provide the College with all the documentation requested, you must provide a written explanation as to why you are unable to do so. The registration committee will review your situation and advise you of acceptable alternatives, such as providing the College with a statutory declaration. A statutory declaration is a written summary of facts that you solemnly declare to be true and then sign in the presence of a notary public or commissioner of oaths.
