Persons who have been granted permanent resident status in Canada but who are not yet Canadian citizens are required to obtain a residency card in order to be readmitted to Canada if they leave the country.

We can help you with your permanent resident card by certifying copies of original documents (such as passports) and photographs as well as witness statutory declarations in lieu of a guarantor that are needed to support your permanent residency card application.

Initial Card, Replacement or Renewal application

This application is provided by Citizenship and Immigration Canada and it includes the following items:

* Guide [IMM 5445]
* Application for a Permanent Resident Card [IMM 5444]
* Use of a Representative [IMM 5476]
* Document Checklist [IMM 5574]
* Supplementary Identification Form [IMM 5455]

(Note: An original must be used. This form contains special features. You will need one Supplementary Identification Form for each person in your application. You must order it from Citizenship and Immigration Canada.)

* Fees Receipt [IMM 5401]

You have 2 options to obtain a Fees Receipt:

1. Online fees payment: Visa, Master Card and American Express credit cards are accepted. Adobe® Acrobat is required to view the receipt. You can pay your fees online.

2. Payment of fees at a financial institution in Canada

You must must obtain an original receipt form [IMM 5401] in order to pay at a financial institution. This form is not available online. You may order an original receipt at the same time as you order the Supplementary Identification Form.
