Ontario Dentists: Bring Your RCDSO Registration Forms to Red Seal Notary

In order to practise dentistry in Ontario, you must be registered with the Royal College of Dental Surgeons of Ontario (RCDSO).
Certificates of Registration
You can obtain a Certificate of Registration by submitting an application form to the RCDSO.
Certificates of Registration are issued in the following categories:
- General (general practitioners)
- Academic (academic appointment at professorial level)
- Specialty
- Education (hospital interns and residents)
- Graduate Student (postgraduate and graduate students)
- Academic Visitor (short-term course instructor)
- Post-Specialty Training
- Short Duration
- Instructional Certificate
Certificates of Authorization for Health Profession Corporations
If you wish to incorporate your dental practice, you will also need to apply for a Certificate of Authorization.
Notarizations, Statutory Declarations & Certified True Copies
You may have to sign your RCDSO application form(s) in the presence of a lawyer, notary public or commissioner of oaths, and you may also have to provide certified true copies of your supporting documents (such as your diploma, NDEB certificate, articles of incorporation, passport, birth certificate, etc.).
