Dental Hygienists: Red Seal Notary Provides Notarizations, Certified Copies & Criminal Record Checks

To become a registered dental hygienist in Ontario, you must apply to the College of Dental Hygienists of Ontario (CDHO).
You can apply if you are:
- a graduate of an accredited or non-accredited dental hygiene programme;
- a past registrant with the College of Dental Hygienists of Ontario (CDHO); or
- a registrant of another dental hygiene regulatory body in Canada and are applying under the Agreement on Internal Trade (AIT).
To obtain the appropriate registration forms for your situation, log in to your account on the CDHO portal.
Red Seal Notary can assist with the following documentation:
- Form AForm A must be completed by the applicant and notarized by a lawyer, notary public or commissioner of oaths.
- Criminal Record CheckYou must provide an original or notarized copy of a Canadian Police Information Centre (CPIC) Criminal Record Report.Red Seal Notary provides Criminal Record Checks at select locations. Please click here for more information on how to obtain a CPIC report.
- Notarized Copy of Degree or DiplomaYou must submit a certified true copy (also known as a notarized copy) of your degree or diploma in dental hygiene. For more information on certified true copies, click here.
- Notarized Copy of NDHCB CertificateIn addition to your degree or diploma, you must also submit a notarized copy of your National Dental Hygiene Certification Board Certificate.
- Other DocumentationYou will also need to provide a certified/notarized copy of one of the following:
- Canadian birth certificate;
- current Canadian passport;
- citizenship card;
- landed immigrant papers; or
- authorization under the Immigration and Refugee Protection Act (Canada).
- Name ChangeIf you have changed your name, you must provide the CDHO with a notarized copy of a document supporting this.
