Statutory Declarations for Physicians and Surgeons in Ontario

Independent Practitioners
If you live in Ontario and you are a graduate of an accredited medical school in Canada or the United States, you will require an Independent Practice Certificate of Registration.
Application forms for independent practice certificates can be obtained through the College of Physicians and Surgeons of Ontario.
Corporations for Practicing Medicine
Under the Regulated Health Professions Act, members of the College of Physicians and Surgeons of Ontario are also permitted to establish corporations for the purpose of practising medicine.
If you have established a corporation, you must apply to the College for a Certificate of Authorization for a Corporation to Practise Medicine.
Statutory Declarations
Whether you are applying for an Independent Practice Certificate of Registration or a Certificate of Authorization for a Corporation to Practise Medicine, your application form includes a declaration that must be executed in the presence of a lawyer, notary public or commissioner of oaths.

Annual Renewals

To avoid having your corporation’s Certificate of Authorization revoked, be sure to bring your Statutory Declaration to any Red Seal Notary location in Ontario for execution prior to the expiration of your certificate.
