In order to practise nursing in Ontario, all nurses must hold a valid Certificate of Registration from the College of Nurses of Ontario (CNO).
If you are an internationally trained nurse applying for your Certificate of Registration, you are required to have your transcripts sent directly to the CNO from the institution at which you studied.
If you are unable to provide this documentation, the CNO will accept a Statutory Declaration instead.
A Statutory Declaration is a written summary of facts that you solemnly declare to be true.
Nurses who wish to incorporate their practice must apply for a Certificate of Authorization for a Health Profession Corporation.
To obtain this certificate, a Statutory Declaration executed by a director of the corporation must accompany the application form. The director must solemnly declare that:
Your Statutory Declaration must be executed in the presence of a lawyer, notary public or commissioner of oaths.
