Ontario Nurses - Statutory Declarations

In order to practise nursing in Ontario, all nurses must hold a valid Certificate of Registration from the College of Nurses of Ontario (CNO).

Internationally Trained Nursing Applicants

If you are an internationally trained nurse applying for your Certificate of Registration, you are required to have your transcripts sent directly to the CNO from the institution at which you studied.

If you are unable to provide this documentation, the CNO will accept a Statutory Declaration instead.

A Statutory Declaration is a written summary of facts that you solemnly declare to be true.

Incorporating a Nursing Practice in Ontario

Nurses who wish to incorporate their practice must apply for a Certificate of Authorization for a Health Profession Corporation.

To obtain this certificate, a Statutory Declaration executed by a director of the corporation must accompany the application form. The director must solemnly declare that:

He or she is a member of the College of Nurses of Ontario and holds a Certificate of Registration.
He or she is a director of the corporation and has the authority to apply for a Certificate of Authorization.
The corporation is in compliance with section 3.21 of the Business Corporations Act (Ontario).
The corporation does not plan to carry on, and will not carry on, any business that is not the practice of nursing or an activity related or ancillary to the practice of that profession.
There has been no change in the status of the corporation.
The information contained in the Application for a Certificate of Authorization that accompanies the Statutory Declaration is complete and accurate.

Executing a Statutory Declaration

Your Statutory Declaration must be executed in the presence of a lawyer, notary public or commissioner of oaths.
