In order to practise optometry in Ontario, you must hold a General Certificate of Registration issued by the College of Optometrists of Ontario.
When you apply to the College for your certificate, you may be asked to provide a number of notarized documents:
Criminal Record Check (CPIC Report)
You must also arrange to have a current CPIC report (criminal record check) submitted to the College

If you choose to form a health professional corporation, you must first incorporate with the Ministry of Government Services under the Ontario Business Corporations Act. Once this is done, you must apply to the College for a Certificate of Authorization. This certificate must be renewed annually.
