Ontario Optometrists: Red Seal Notary can assist with your notarization and CPIC requirements

In order to practise optometry in Ontario, you must hold a General Certificate of Registration issued by the College of Optometrists of Ontario.

Notarized Documents

When you apply to the College for your certificate, you may be asked to provide a number of notarized documents:

  1. Your application form.
  2. Include a notarized passport photo – bring the application form with you to the notary with your photo.
  3. Provide a notarized copy of your Doctor of Optometry degree and all other degrees, certificates and fellowships.
  4. Provide a notarized copy of your birth certificate, Canadian passport or citizenship identification card.
  5. If you have permanent resident status or authorization to practise optometry under the Immigration and Refugee Protection Act, you must provide a notarized copy of your PR card or authorization.
  6. If you completed the Canadian Standard Assessment in Optometry (CSAO) or the Canadian Assessment of Competence in Optometry (CACO), but did not apply for your certificate in Ontario at that time, provide a notarized copy of your results.

Criminal Record Check (CPIC Report)

You must also arrange to have a current CPIC report (criminal record check) submitted to the College

Incorporating Your Practice

If you choose to form a health professional corporation, you must first incorporate with the Ministry of Government Services under the Ontario Business Corporations Act. Once this is done, you must apply to the College for a Certificate of Authorization. This certificate must be renewed annually.

The process includes the following requirements:

  • a declaration executed by each shareholder and director of your corporation;
  • a copy of your Certificate of Incorporation (including the Articles of Incorporation);
  • a form detailing the corporation, its directors and shareholders;
