Medical Radiation Technologists of Ontario: Notarization Requirements

To work as a medical radiation technologist in the province of Ontario, you must be registered with the College of Medical Radiation and Imaging Technologists of Ontario (CMRITO).

You can apply for a CMRITO Certificate of Registration if you meet one of the following criteria:

  • you have successfully completed an approved program in Ontario
  • you are currently registered by a regulatory authority in another province and the labour mobility provisions of the Regulated Health Professions Act (RHPA) apply to you
  • you have successfully completed an educational program in Canada and/or have been employed as an MRT in a province other than Ontario
  • you have successfully completed an educational program in medical radiation technology outside of Canada

Notarization Requirements for Certificates of Registration

Ontario Applicants

If you are an Ontario applicant, you must prove successful completion of a medical radiation technology program by:

  1. completing Part B of the application form and sending it to your program director for his or her signature, or
  2. providing a notarized copy (certified true copy) of your certificate, diploma or degree, or
  3. providing an original or notarized copy of your academic transcript.

Canadian Applicants

If you are a Canadian applicant (outside of Ontario), you must:

  1. attach a notarized copy (certified true copy) of your certificate, diploma or degree, or
  2. attach an original or notarized copy of your academic transcript, or
  3. provide an original letter from your educational institution confirming successful completion of the program.

International Applicants

If you are an international applicant you must:

  1. attach a notarized copy (certified true copy) of your degree, certificate or diploma in medical radiation technology or provide an original letter from your educational institution confirming successful completion of the program;
  2. submit an original or notarized copy of your academic transcript.

If you have completed a pre-requisite post-secondary educational program or taken courses prior to entering your program in medical radiation technology, you must also:

  1. attach a notarized copy (certified true copy) of your degree, certificate or diploma or provide an original letter from your educational institution confirming successful completion of the program;
  2. attach an original or notarized copy of your academic transcript pertaining to the pre-requisite post-secondary educational program or courses.

If you are unable to provide original or notarized copies of your credentials, your academic transcripts, or an official curriculum of your programs, the registration committee of the CMRITO may accept other evidence. Contact the CMRITO for more information.

Notarization Requirements for Certificates of Authorization

If you wish to incorporate your practice, you will require a Certificate of Authorization, and you must provide the following supporting documents with your application form:

  1. a notarized copy (certified true copy) of your Certificate of Incorporation (including the articles of incorporation);
  2. a notarized copy (certified true copy) of every certificate of your corporation that has been endorsed under the Ontario Business Corporations Act;
  3. the statutory declaration of a director of your corporation executed not more than 15 days before the application is submitted.

Your Certificate of Authorization must be renewed annually.
